Client Forms
To get started on your financial journey with Sound Financial Planning Inc., please download the following client forms and follow the instructions listed below to submit them to our company.
Instructions to Download and Submit Forms using Microsoft Word
Note: If you do not have Microsoft Word on your operating system. Please continue to scroll down this page for instructions on how to submit forms by using a PDF file.
Step 1: Click on the following three forms and select “Open.”
Initial Interview Form (Word File)
Financial Satisfaction Survey (Word File)
Life Transitions Survey (Word File)
Step 2: Once the document is downloaded, please type out the answers to the questions that we have listed on the forms. (If you prefer to hand write your answers please print the forms and fill in your information.)
Step 3: Once you have completed all the forms, please submit them to our office. If you have typed your answers into the Microsoft Word documents, you can save each file to your computer and email the saved forms to Bill Morrissey as attachments at wtmorrissey@soundfinancialplanning.net. If you have hand written your answers, you may fax the forms to our company at (360) 336-6528.
Instructions to Download and Submit Forms if you do NOT have Microsoft Word
Step 1: If you do not have Microsoft Word on your computer then you may download the PDF copies of each of the three files here by clicking on each file and selecting “Open”.

Step 2: Once the PDF copy of each file is open, print the form and fill out the information by hand. Please write so that the form can be easily read by our company.
Step 3: Once you have completed the form, please fax all three forms to our company at (360) 336-6528.
